A healthcare organization is looking for a personal counselor to provide intensive support to adult residents. You will work with them to develop plans for goals in various areas of life, aimed at strengthening their self-reliance and restoring a normal life. The organization offers housing, guidance, and care to people dealing with psychiatric vulnerability, addiction, homelessness, or a combination thereof.
The location includes two departments, one of which is for participants with a mild intellectual disability (MID). You will work in a multidisciplinary team.
What you do
- Inventorying the participants' need for help
- Conducting counseling sessions with professional guidance
- Drawing up care plans, monitoring progress, and evaluating assistance
- Administrative responsibility, such as correctly registering data and monitoring indications
- Contributing to psychosocial support in all areas of life, focused on recovery and self-reliance
- Increasing the resident's insight into their own behavior
- Close collaboration with chain partners and participation in multidisciplinary consultations
- Requesting and monitoring indications and follow-up trajectories
Profile
- Completed HBO education in social work or similar
- Insight into the social map of the region
- Experience in a similar role within an intramural setting
- Knowledge of methodologies, techniques, and developments in care
- Ability to work independently
- Proficiency in the Dutch language, both written and spoken
- You are a Christian (Catholic, Protestant, etc.) and can explain what that means to you
Practical
- Availability on Thursdays is required for fixed consultation moments and multidisciplinary meetings
- Availability on Mondays is a plus
