Clinical Recovery Case Manager
Support clients in their recovery process. As a case manager in clinical recovery, you treat and guide clients with severe psychiatric disorders (SPD) whose functioning is limited, preventing them from living independently (yet). Together with fellow case managers, you are responsible for the care coordination of clients in various domains, with the aim of improving their quality of life.
What you will do.
- Signal and investigate the client's clinical picture
- Signal somatic complications and other changes in the overall condition
- Write recovery/treatment plans, which you periodically discuss with the treatment team
- Contact caregivers, family, and social organizations on behalf of the client
- Adjust care in consultation with the treatment team and the nursing team, and ensure its correct implementation
- Ensure complete and easily transferable documentation and reporting
Profile
You can sharpen your substantive knowledge and affinity by acting as a focal point for a specific area of attention (e.g., psychosis, addiction, family policy, or somatic issues). You are open to learning, naturally curious, and well able to empathize while also relativizing. You can handle chaos and unpredictability.
What they ask for.
- A higher professional education (hbo) diploma as a nurse or social worker
- Familiarity with recovery-oriented work and the ART methodology is a plus
- Experience with addiction and personality issues is a plus
