The regional teams are part of a specialized unit. From various regions, outpatient care is offered to adults and the elderly, including FACT teams, regional teams, cluster teams, ART Wonen, and outpatient guidance.
You will work in a dynamic environment where collaboration and flexibility are central. The region is characterized by a mix of urban and more rural areas, which ensures a wide variety of clients and care needs. We work closely with partners in the neighborhood, such as general practitioners, municipalities, and welfare organizations, to quickly switch and organize appropriate care.

- Early detection and prevention
- Outreach support is only deployed when necessary, and guidance takes place on-site as much as possible.

What you do
As a Coordinating Nurse/Nursing Case Manager, you will:
- Be responsible for your own caseload
- Support clients in dealing with psychiatric and psychosocial problems
- Provide recovery-oriented care and keep an eye on somatic health
- Work methodically and actively contribute to quality improvement
- Have intensive contact with colleagues and network partners inside and outside the organization
- Work closely with lead practitioners, such as psychiatrists, nursing specialists, and GZ psychologists
- Offer group treatment together with colleagues from other regional teams
You have a versatile role in which you work independently and are an important connecting factor within the team and the network around the client.
What they ask
- A completed HBO (higher professional education) degree in Nursing and BIG registration, or HBO Social Work with GGZ agoog registration
- Extensive experience in psychiatry, preferably with the EPA target group
- Knowledge of psychiatric and somatic problems
- A proactive attitude and strong communication skills
- The ability to work methodically and maintain an overview in complex situations
- The motivation to work directly with clients on recovery, in addition to coordinating tasks
Practical
- Salary according to CAO-GGZ FWG 50 (€3,560 - €4,716 gross per month based on 36 hours)
