Case Manager

A healthcare organization is looking for a case manager for a specialized team. You will treat clients with severe psychiatric complaints and manage your own schedule. The position offers a varied and meaningful job in a professional working environment.

What you will do.

• Provide outpatient treatment to clients between 18 and 65 years old with complex, long-term psychiatric problems.
• Work towards recovery, with an eye for the person behind the diagnosis, where psychological, social, and societal challenges also play a role.
• Offer recovery-oriented, network care that aligns with the client's life.
• Work proactively and multidisciplinarily with colleagues, including psychiatrists, specialized nurses, and peer support specialists.
• Be responsible for the treatment and support of clients in your own caseload.
• Maintain contacts with relatives and chain partners.
• Work independently, but always be able to count on the support and cooperation of colleagues when needed.

What they ask

• Completed HBO Nursing diploma.
• Several years of work experience as a therapist with EPA (severe and persistent psychiatric disorder) clients.
• Able to independently carry out care coordination for your own caseload.
• Good at administrative planning and organization.
• Crisis-resistant and able to independently assess what care is needed.
• In possession of a driving license and a car.
