Social Worker

Guidance for young people aged 12 to 18 with externalizing behavioral problems or sexually transgressive behavior and their network within a residential treatment setting at a healthcare institution.

What you will do.

• Conduct social research and propose appropriate services, tailored to the patient and their context.
• Guide patients towards a feasible discharge situation and ensure the necessary follow-up care.
• Promote the psychosocial well-being of patients and collaborate multidisciplinarily.
• Prepare clear social reports and advice, also for external bodies.
• Maintain contacts with external aid workers and organizations for referrals and collaboration.
• Coordinate individual files and guide patients and their context in socio-community care.

What they ask

• Diploma: Bachelor in Social Work or Social Nursing.
• Knowledge of the social assistance field, social provisions, rights research, and social legislation.
• Interest in ethical issues and psychological disorders.
• Good basic knowledge of Microsoft Office and other IT applications.
• Excellent communication skills in Dutch.
• Good intercultural and administrative skills.
• Independent, structured, able to set priorities, and solution-oriented.
