What you do
- Perform (care) diagnostics and discuss clients in multidisciplinary consultations.
- Implement the guidance plan and conduct evaluation and results discussions.
- If necessary, request care or guidance from relevant authorities and ensure coordination.
- Be part of a specialized department.
- Maintain contact with referrers and inform them about diagnostics, treatment/guidance plans, progress, and developments.
- Provide advice and consultation to various 1st and 2nd line institutions regarding clients' psychogeriatric issues.
- Participate in preventive, informative services.
Profile
- You support person-centered care, are enthusiastic, collegial, and have a strong sense of responsibility.
- HBO-V diploma or a social-societal education at HBO level.
- Completed dementia case management training or willingness to follow it.
- Extensive professional knowledge and experience in psychogeriatrics.
- Knowledge of the social map of the region.
- Knowledge of relevant laws and regulations is necessary (e.g., WLZ, ZVW, Wmo, Wzd).
- Driving license and car to travel to different locations.
