A healthcare organization is looking for a care manager / lead practitioner for one of its clusters within a clinical division. This division offers specialized psychiatric care to people with complex psychiatric problems. The focus is on high-quality, innovative, and recovery-oriented care, with short lines of communication and strong mutual trust. You will have the freedom to guide the development of care based on your expertise.
What you will do. You will primarily be active as a lead practitioner within the multidisciplinary team of 1 or 2 departments in the cluster. Additionally, you will fulfill the role of care manager. You will remain close to clients and colleagues and bring your experience from daily practice into the development and quality of care.
Your responsibilities will include:
- contributing to diagnostics, treatment, casuistry, and treatment policy
- monitoring and improving the quality and content of client care
- further developing treatment offerings, collaboration, and care processes
- creating a positive work and training environment
- connecting and guiding professionals
- collaborating with fellow managers, management, and chain partners
