Case Manager

As a Wmo case manager at a municipal organization, you are the first point of contact for residents with complex problems in multiple areas of life, specializing in single individuals.

  • You map their situation, draw up Wmo advice, and manage the support process.
  • You are not a direct care provider, but you manage and connect residents with the right support.
  • You work closely with care providers, social shelters, and other chain partners.
  • You monitor progress, maintain an overview, and ensure that residents can take steps towards recovery and a suitable housing solution.

What you will do.

  • You have weekly counter services where you receive residents who have become homeless, often with multi-faceted problems.
  • You investigate the situation, inventory the circumstances, and determine what support is needed, including social shelter and/or guidance.
  • You process all this into Wmo advice and guide vulnerable residents to the right care and support for a good start to recovery.
  • Based on the Wmo advice, you create a joint action plan, including actions that residents can undertake themselves.
  • You ensure a personal plan with as much freedom of choice as possible through short lines of communication with professionals inside and outside the organization.
  • You can oversee the big picture and also pay attention to (seemingly) small details that can be of great significance for someone's well-being and future prospects.
  • The majority of your time will be spent on administrative tasks.

  • A large dose of creativity and flexibility, given the complex demand for help and limited resources.
  • Experience working with and for people living on the margins of society.
  • Knowledge of the care chain and familiarity with the specific problems of homelessness.
  • Ability to quickly make a good inventory and assessment of problems in various areas of life.
  • Good at handling work pressure and maintaining calm in a hectic work environment, taking control of your own caseload.
  • Ability to switch between policy language, care provider language, and the language of the resident.
  • Motivation to take the lead, even with administrative tasks.
  • Familiarity with mental challenges and resilience to deal with them.
  • A personal drive to make a difference in people's lives daily, and to make maximum use of the strength of people themselves and their immediate environment.

Profile

  • HBO diploma (higher professional education), for example Social Work.
  • Knowledge of the social domain and preferably of the Wmo.
  • Minimum of 3 years of work experience with vulnerable individuals.
  • Experience with setting Wmo indications is a plus.
