A healthcare organization is looking for an ambulatory counselor for the IB department. You add value to clients by structuring their lives and supporting them on their path to recovery.
What you will do.
- You have a flexible schedule and work with diverse clients.
- You collaborate with healthcare professionals in a dynamic environment.
- You will be part of a dedicated team of 18 colleagues.
- There are monthly meetings and telephone consultations with colleagues.
- You guide people who need support for their recovery, for example with psychological or addiction problems, or preventively.
The Work at a Glance.
- coordinating services for the client at one of the locations or at the client's home;
- guiding the client with support questions in all areas of life, such as housing, daily activities, social contacts, and finances;
- monitoring and applying for funding for the trajectories;
- building a strong network, so you can quickly help clients;
- participating in multidisciplinary meetings.
Profile
- You can motivate, coach, and stimulate, even when things are difficult.
- You have a completed HBO-level education; preferably SPH, MWD, or Social Work.
- You belong to the CONO professional list.
- A positive Certificate of Conduct (VOG) is required.
- Reference checks and diploma verification are part of the application procedure.
