As an ambulant counselor, you work independently and visit clients at home. You help them and their relatives to gain control over their daily lives. No day is the same, as every situation is unique. You offer practical support, are a listening ear, and help clients gain control over issues such as health, work, finances, housing, and social contacts.

What you do

  • You manage the case and are primarily responsible for the client's care file.
  • You draw up care plans, monitor progress, evaluate results, and adjust where necessary.
  • You look beyond today's request for help and identify what a client needs to move forward.
  • You maintain an overview when situations are complex or involve multiple parties.
  • You are the fixed point of contact for municipalities and other involved organizations and maintain contact regarding indications, decisions, and the progress of the guidance.
  • You build trust, set boundaries when necessary, and motivate clients to get the most out of themselves.
  • You are an important sparring partner for colleagues, contribute to the quality of guidance within the team, and ensure that clients and their relatives can count on appropriate support.

Profile

  • You hold a higher professional education (hbo) diploma in Healthcare and Welfare or Human and Society, such as SPH, Social Work, Nursing, or MWD.
  • You are interested in methodical work and competency-oriented guidance for clients with brain injury.
  • You have work experience in a similar position.
  • You possess a B driving license and your own transportation.

Practical aspects.

  • You work with clients in a specific municipality and are part of one of two self-organizing teams.
  • Each team consists of 12 to 14 colleagues who provide ambulant guidance or work within day care in a guidance center.
  • Although you work independently, the teams consciously seek each other out.
  • Sometimes you guide a client together with a colleague from day care, so you can work together towards an independent life.
  • You have a lot of autonomy and guide clients at their homes where the guidance request varies in complexity.
  • Once every three weeks, the teams meet to stay informed, help each other, and utilize each other's qualities.
  • In between, there is easy contact with colleagues from your own and other teams.
  • Upon starting, you will receive a buddy and follow an onboarding program.
  • You support people in reshaping their lives after brain injury.
  • Great importance is attached to a healthy work-life balance.
