A healthcare organization is looking for a social worker for its clinics. You empower clients so they can regain control of their lives independently.
What you will do.
- In individual contacts with clients, you primarily focus on the quality of life and restoring the client's autonomy.
- You map out the social and societal situation in which someone finds themselves.
- You have a signaling function and are able to react professionally to unexpected situations.
- You are part of a multidisciplinary team and work for various departments.
- Specific tasks include:
- (Re)insuring clients.
- Material assistance (starting benefits, budget management, administration, support with debt problems).
- Assistance with housing problems, day activities, (volunteer) work.
- Coordination of necessary care: mediation between clients and agencies and collaboration with the clinical team and involved (ambulatory) therapists.
Profile
- You hold a higher professional education (HBO) diploma in social work or social services (with mental health registration).
- You enjoy working independently and have a flexible attitude.
- A positive Certificate of Conduct (VOG) is required for this position.
Practical
- A varied and meaningful job in a professional work environment.
- One-year contract with the prospect of permanent employment.
- Salary according to FWG 55.
- Employment conditions in accordance with the GGZ collective labor agreement.
