A care provider has been active for over 20 years as a fully ambulatory care provider, specializing in (characteristics of) autism. The organization serves anyone who can benefit from an autism-friendly approach and also offers guidance for giftedness, acquired brain injury, and AD(H)D. The organization is innovative and growing, with equal attention for employees and clients, and a flat structure with regional, independent teams that provide broad care, both individually and in groups, physically and digitally.
What you do
- You work both ambulant and remotely with a fixed group of 12-24 clients (children, young people, adults) with an indication from the partnership/WMO/Jeugdwet or WLZ, in accordance with the professional code.
- You collaborate with a lead coach who conducts initial interviews, draws up care plans, performs evaluations with clients, and can provide SKJ supervision where necessary.
- You have fixed moments for work meetings, client meetings, and intervision with colleagues.
- You work intensively with the client's network; at home, at school, and at work.
- Guidance moments usually last 1.5 to 2 hours, once a week or every other week.
- You help clients with routines for self-care and energy management, administration, and practical situations regarding social expectations.
- You accompany clients during shopping and other daily activities.
- You conduct online appointments and discuss eHealth programs.
- In addition to client contact, you conduct discussions with municipalities, mental health institutions, psychologists, and other individuals in your client's network.
- You update administration, write care plans and evaluations (with support), research information, or consult with a colleague.
