This position is within an Acute Geriatrics department of a healthcare organization. You will be responsible for the total care of elderly patients with complex and often multiple care needs. The environment is characterized by clinical complexity and person-centered care, where clinical reasoning, empathy, and personalized care are central.
What you do
- Provide integrated nursing care to geriatric patients in an acute setting
- Observe and monitor physical, cognitive, and psychosocial developments
- Early detection of risks such as delirium, falls
- Actively collaborate within a multidisciplinary team (doctors, physiotherapists, occupational therapists, social workers, etc.)
- Involve and guide family and informal caregivers
- Contribute to discharge planning and continuity of care
What they ask
- Bachelor's or HBO5 Nursing Diploma
- At least 2 years of relevant work experience as a nurse
- Experience within a hospital setting (required), preferably in a department with complex care needs
- Experience in geriatrics is a strong asset, but not a must
- Great empathy and affinity with elderly patients
- Strong observational skills and clinical insight
- Team-oriented, communicatively skilled
