Social Worker Adults Case Management and Intervention
As an Adult Social Worker - Case Management and Intervention, you support residents in vulnerable and complex situations. Often, safety issues, social disruption, or acute problems are at play. You take action when necessary, carefully assess the situation, and take the lead where required. You work from one of the four district teams of a social organization and are a specialist in safety and complex issues within specific target groups.
You work with various special target groups, including homeless residents, residents with misunderstood behavior, and residents causing severe housing nuisance or social unrest. You also provide aftercare to residents released from detention and support residents who are (presumably) victims of human trafficking or (criminal) exploitation. Additionally, you work with victims of severe domestic violence or honor-related violence and with residents facing lease termination, eviction, or worrying rent arrears.
Working with these target groups requires careful analysis, quick decision-making, and intensive collaboration with partners such as municipal trajectory management, housing corporations, police, child protection services, social shelters, and a reintegration partner. You are an important link between residents, colleagues, and collaborating partners, committed to appropriate interventions, safety, and stability.
