- Good collaboration between teams, with chain partners and with the client's support system, is very important.
- The vision focuses on positive health, recovery-oriented, and network-oriented care.
- Work is carried out with FACT (Flexible Assertive Community Treatment) teams within a care program for adult care.
- FACT is a method for outpatient, outreach treatment and guidance, as much as possible in the client's own environment.
- In case of crisis, care can be easily intensified through the joint efforts of multiple practitioners, to prevent or shorten hospitalization.
What you will do.
- Together with a psychiatrist and nursing specialist, you will be the lead practitioner of the team.
- You will be instrumental in determining the diagnostics and treatment of clients and colleagues within the multidisciplinary team.
- You will manage your own caseload, coordinating treatments with other disciplines such as (social psychiatric) nurses, social workers, and peer experts.
- You will work both internally and externally, as required by the treatment.
- You will actively contribute to the team climate and the further development of the treatment offerings within FACT.
Profile
- A proactive colleague with a good amount of work and life experience, a sense of humor, and focused on collaboration.
- Possesses relevant education(s) and associated registrations.
- Creative and critical, actively contributes to policy discussions.
- Is energized by the treatment approach and is responsible for building and maintaining team spirit.
- BIG-registered and holds a GZ-Psychologist registration.
- An experienced professional who can quickly integrate into FACT team processes and make good decisions with the team and fellow lead practitioners.
