As a case manager, you are the linchpin in the care for people with dementia or other cognitive challenges and their loved ones, coordinating care and providing tailored support. The district nursing service of a healthcare organization is expanding and is looking for an OGGZ case manager. You will be part of a district nursing team and work within a small, dedicated case management team.
What you will do.
- You coordinate and direct care for clients with OGGZ issues and memory complaints
- You play a connecting role between the client, network, and healthcare professionals
- You provide expert support to the team, clients, and their loved ones, helping them cope with the challenges that dementia brings
- You develop and implement individual care plans in close collaboration with a multidisciplinary team and ambulatory supervisors
- You provide clear information and education to clients and informal caregivers about dementia, available services, and support options
- You guide clients and informal caregivers in making well-informed choices
- You maintain intensive contact with general practitioners, geriatricians, district nurses, fellow case managers, the municipality, and other care institutions to create a cohesive and effective care network
- You closely monitor client progress, regularly evaluate care plans, and adjust them based on changing needs and circumstances
- You carefully and timely document all care interactions and decision-making
Profile
- HBO nursing diploma and valid BIG registration
- Post-HBO diploma in dementia case management or willingness to obtain it (training starts in October)
- Experience with indicating and applying for CIZ indications
- Driving license
- Ability to deal with behavior appropriate to the OGGZ target group
- Christian identity (Catholic, Protestant, etc.) and willingness to explain what that means to you
Practical
- During the initial period, you will be trained in the district
- During the training (if not yet completed), you will work in the district, which may temporarily involve irregular shifts
- Possibility to combine this position with work as a nurse in district nursing
