WMO Consultant

An experienced WMO consultant is sought for a Social Participation team. The role focuses on arranging appropriate care and support for residents, with an eye for the balance between support, care, safety, and liveability. The goal is to find sustainable solutions in collaboration with various parties.

What you will do.

• Answering resident questions and reports; initiating research and intake.
• Conducting home visits and performing on-site investigations.
• Consulting and coordinating with care providers and suppliers of aids.
• Explaining to residents what they can do themselves and what the organization can offer.
• Organizing and implementing help and support where necessary.
• Monitoring and adjusting agreements and care trajectories once the solution has been implemented.
• Contributing to and maintaining the necessary administration and plans.
• Starting with a team meeting to discuss and distribute incoming reports.
• Collaborating with colleagues from, for example, debt counseling, youth care, or the social base for case solutions.
• Consulting with a Dementia case manager about supporting a couple where one partner has dementia; drawing up a plan with concrete agreements and goals.
• Home visit to an elderly client to discuss the request for help regarding daily routine and independent living.
• Concluding the day by writing down agreements in a plan and organizing the administration.

What they ask

• CV of maximum 3 A4 pages.
• Minimum three years of experience as a WMO consultant. Concretely mention and describe assignments/projects demonstrating this experience, including the name of the organization(s).
• Relevant HBO degree. State the name of the educational institution, the location, the specialization, and the graduation year.

Preferences.

• Demonstrably trained in the field of WMO.
• Three years of recent work experience in the field of WMO.
• Demonstrable experience in the field as described.
• Three years of demonstrable experience within a government organization.
