This role involves working as a Caregiver IG within a specialized team that provides nursing home care at home to clients with a WLZ indication. You will build a close bond with clients and their relatives, work independently, and contribute daily to maintaining independence, self-direction, and quality of life.
What you will do.
- Provide support with personal care.
- Perform nursing technical procedures.
- Monitor clients' health and situation and respond promptly to changes.
- Collaborate with relatives, informal caregivers, therapists, and other healthcare professionals to ensure clients can live safely and comfortably at home.
- Contribute to discussions on appropriate care and current care plans.
- Supervise students or interns where necessary.
- Travel frequently within the service area.

- Diploma as Caregiver IG level 3.
- Or you are a 'Helpende Plus' and wish to grow into a Caregiver IG.
- Or you have an older care diploma and wish to return to healthcare.
- You enjoy working independently but also actively seek collaboration with colleagues and other disciplines.
- You are attentive to changes in clients' health or situation.
- You consider person-centered care to be self-evident.
Practical
- Use of an e-bike or company car during shifts.
- Provision of a work phone.
- Working hours are between 07:00 and 23:00; no night shifts.
- A start time from 07:30 is negotiable.
- Possibility to combine with work within an internal flexible pool.
