What you will do.
- Examine, treat, and guide patients with complex and chronic care needs.
- Develop, implement, and evaluate treatment plans.
- Collaborate with general practitioners, specialists, practice assistants, and other healthcare professionals.
- Participate in multidisciplinary meetings and regional care networks.
- Advise on exercise, lifestyle, and self-management.
- Contribute to innovative projects in primary care.
- Treat patients with cardiovascular and pulmonary conditions, Multiple Sclerosis (MS), frail elderly, complex chronic conditions, and rehabilitation and fall prevention issues.
Profile
- Physiotherapist and BIG-registered.
- Registered in the Quality Register (Keurmerk/KNGF) Physiotherapy.
- Works independently and takes responsibility for patients.
- Affinity with chronic care.
- Involved, patient-oriented, and strong in communication.
- Enjoys working collaboratively within a multidisciplinary network.
- Actively contributes to quality and innovation within the practice.
Preferences.
- Master's in Geriatric Physiotherapy.
- Master's in Cardiovascular and Pulmonary Physiotherapy.
- Master's in Psychosomatic Physiotherapy.
- Affiliation with Chronisch ZorgNet (heart, lung, and vascular).
- Affiliation with the MS network.
- Experience with Intramed Online.
- Experience with reporting (clinimetry) at certified practice level.
