You contribute to the recovery of people with serious psychological disorders and help build future-oriented, network-focused mental health care within local teams. A new way of working brings together ambulatory psychiatry, specialized outpatient care, elderly psychiatry, and youth expertise. Care is brought closer to the client and organized integrally, recovery-oriented, and connected to the network.
The role is dynamic and involves home visits, consultations with general practitioners, municipalities, and other chain partners.
What you will do.
- Conduct recovery-oriented intake interviews to understand the client and their request for help.
- Develop, implement, and evaluate recovery plans.
- Ensure care coordination within your caseload.
- Maintain contact with care providers, relatives, and social partners.
- Ensure complete and transferable documentation and reporting.
- Actively contribute to team development and the substantive evolution of the team.
- Collaborate within a team of approximately 30 colleagues.
Profile
You are a committed professional who is resilient and energized by ambulatory work in a complex and sometimes unpredictable setting. You can handle dynamism and switch flexibly between different roles. An affinity with the vision of recovery-oriented work and a natural ability to work in a developing environment are important. A proactive, positive, flexible, and transparent attitude is required.
What they ask
- A diploma in HBO nursing (Hbo-V) or Social Work.
- Preferably, experience in ambulatory mental health care.
- Experience with depot medication is a plus.
- A B driving license and access to a car.
