As an elderly care supervisor at a care institution, you guide residents with complex psychological and/or psychosocial problems in their daily lives.
- You observe behavior, conduct anamnesis and research, and translate your findings, together with the client and their network, into a suitable sub-plan.
- This is discussed within the multidisciplinary treatment team.
- You methodically and purposefully guide clients in areas such as self-management, daily functioning, and resocialization.
- In acute or crisis situations, you remain calm and act appropriately.
- You engage a nurse or behavioral expert where necessary and also provide somatic care if required.
- Additionally, you coordinate and monitor the guidance.
- You represent the client in consultations with other care providers, ensure proper handover, and report carefully.
- You also contribute to improving working methods and care programs and organize activities that contribute to the well-being of clients.
Profile
- You hold an MBO 4 diploma in Social Care.
- You have extensive experience with clients with complex psychiatric and/or psychosocial problems.
- You are familiar with relevant methodologies, protocols, and the social map, and have experience in coordinating care.
- Apply methodical working according to applicable protocols and the social map.
- Coordinate and monitor care for clients.
- Work independently, set priorities, and take responsibility.
- Remain calm and decisive in acute and crisis situations.
- Communicate clearly and know how to motivate and set boundaries for clients.
- Timely observe and signal risks and changes.
- Work systematically and carefully, properly documenting agreements and progress.
