Care Coordinator
As a Care Coordinator, you are central to the care for people with acquired brain injury (ABI). You coordinate the care process, monitor quality, and ensure that residents receive the support they need to lead their lives as independently as possible.
What you do
- You are the on-site point of contact for residents, colleagues, and collaboration partners.
- You play an active role in involving and coaching your team in the implementation of care plans.
- You provide direction to your role and shape team collaboration.
- You contribute to the design and organization of team meetings, resident gatherings, and other initiatives that contribute to a stable and pleasant living environment.
- In close collaboration with nurses and behavioral scientists, you determine who takes on which tasks, without performing medical procedures yourself.
- You respond quickly to changing care needs and ensure that all agreements are carefully recorded in the care file.
What they ask
- A higher professional education (HBO) diploma in Social Work, Social Pedagogical Assistance, Nursing, or Social Work.
- At least 1 year of work experience in healthcare.
- Willingness to occasionally work irregular shifts in consultation.

Practical
- You primarily work at a residential location, the home of 16 people with ABI, each with their own apartment.
- There is a committed and pleasant atmosphere, with shared coffee and meal times and activities from a guidance center.
- You are part of a dedicated professional group with five care coordinators, two behavioral scientists, two team coordinators, four nurses, and a lead nurse.
- You have a meeting every two weeks and intervision every eight weeks.
