OGGZ Case Manager

As a case manager, you are the central figure in the care for people with dementia or other cognitive challenges and their relatives, coordinating care and offering tailored support. You will be part of a district nursing team and work within a small, dedicated case management team.

What you will do.

  • You provide expert support to both the team and to clients and their relatives, helping them cope with the challenges that dementia brings
  • You develop and implement individual care plans, working closely with a multidisciplinary team and ambulatory supervisors to ensure optimal care
  • You provide clear information and guidance to clients and informal caregivers about dementia, available services, and support options, and guide them in making well-informed choices
  • You maintain intensive contact with general practitioners, geriatricians, district nurses, fellow case managers, the municipality, and other care institutions to create a cohesive and effective care network
  • You closely monitor the progress of clients, regularly evaluate care plans, and adjust them based on changing needs and circumstances. You carefully and timely document all care interactions and decision-making

Profile

  • You have a HBO nursing diploma and possess a valid BIG registration
  • You have a post-HBO diploma in dementia case management or are willing to obtain it (the training starts in October)
  • You have experience with indicating and applying for CIZ indications
  • You possess a driving license
  • You can handle behavior appropriate for the OGGZ target group
