As a casemanager, you are the linchpin in the care for people with dementia or other cognitive challenges and their relatives, coordinating care and offering tailor-made support. You will be part of a district nursing team and work within a small, dedicated casemanagement team currently consisting of two colleagues.
What you do
As a casemanager, you coordinate and direct care for clients with OGGZ issues and memory complaints, playing a connecting role between the client, network, and healthcare professionals.
- You provide expert support to both the team and to clients and their relatives, helping them cope with the challenges that dementia brings
- You develop and implement individual care plans, working closely with a multidisciplinary team and ambulatory supervisors to ensure optimal care
- You provide clear information and education to clients and informal caregivers about dementia, available services, and support options, guiding them in making well-informed choices
- You maintain intensive contact with general practitioners, geriatricians, district nurses, fellow casemanagers, the municipality, and other care institutions to create a cohesive and effective care network
- You closely monitor client progress, regularly evaluate care plans, and adjust them based on changing needs and circumstances. All care interactions and decision-making are carefully and timely documented
Profile
- You have an HBO nursing diploma and a valid BIG registration
- You possess a post-HBO diploma in dementia casemanagement or are willing to obtain one
- You have experience with indicating and applying for CIZ indications
- You have a driver's license
- You can handle behavior appropriate to the OGGZ target group
- You are a Christian (Catholic, Protestant, etc.) and can articulate what that means to you within a Christian healthcare organization
Practical
- As a casemanager, you work regular office hours
- In the initial period, you will be trained in the district to get to know the team and the clients well
- It is possible to combine this role with work as a nurse in district nursing
