POH Elderly Care
You will contribute to the development of elderly care within a primary care organization. You will work in a dynamic environment where you can continuously develop and collaborate with colleagues at various health centers.
What you do
As a POH Elderly Care, you will engage with elderly individuals and their families at an early stage to provide appropriate assistance. This involves patients with physical, cognitive, and psychological fragility due to aging, such as (early-stage) dementia, loneliness, or physical discomforts. You will map out the patient's situation and draw up a care plan. It is important to build a relationship of trust with the elderly, their social environment, and other involved healthcare providers.
You will take the lead in the process and involve the immediate environment, the social domain, and other care disciplines. Your activities include:
- Providing consultations, both at the practice and at the homes of elderly individuals, including physical examination.
- Coordinating and managing care, including drawing up individual care plans, multidisciplinary consultations, and guiding chronic care.
- Maintaining and expanding a network within the social domain and connecting 0th, 1st, and 2nd line care.
- Monitoring medication safety, guiding medication, and facilitating advance care planning for the general practitioner.
Profile
The POH Elderly Care role is evolving, and you will play an important part in its implementation. You possess strong organizational and social skills. You enjoy pioneering and optimizing processes. You have a proactive and enthusiastic attitude and know how to motivate others. You bring attention to the care for (vulnerable) elderly individuals. As care is partly provided at people's homes, you must have a driving license and your own transport. You have completed one of the following qualifications:
- A completed POH Elderly Care training at NLQF-6 level.
- HBO-V in combination with the POH Elderly Care training NLQF-6 or the desire to follow this training.
- POH Somatics HBO-V qualified in combination with the POH Elderly Care training or the willingness to follow it (MBO qualified nurses have an NLQF-6 level qualification, or are already admitted to the training).
- Nursing Specialist.
- Physician Assistant with additional training and/or experience in Elderly Care.
- A minimum of 2 years of relevant work experience within elderly care.
The Promedico ASP system is used.
