As a Social Worker, you monitor cases from a crisis team or from other colleagues. You follow the case for an average of 1 year to see if the home situation is going well and continues to go well. You have at least 2 monitoring moments per year (after 3 and 12 months) with telephone contact with the aid agency and the directly involved person(s). You assess whether the safety conditions are met and whether they lead to stable safety. You look for structural improvement that breaks the cycle of violence.
If necessary, you have more frequent contact and monitor longer. Attention is also paid to any trauma incurred and the deployment of recovery-oriented care. You are also the point of contact for the local field and other chain partners. You schedule or join round table discussions. You adjust conditions, escalate towards child protection services, or close the case when stable safety can be spoken of.
You have a coordinating and advisory role and are a central figure between adults, children, the local field, and various youth protection organizations.

- Completed HBO (higher professional education) degree in Social Work or similar (MWD, SPH, Pedagogy, or HBO nursing).
- Experience in working with complex family situations.
- SKJ or BIG registered or meets the requirements for registration.
- Willingness to perform on-call duties.
- Knowledge of complex problems and their consequences for a household (addiction, aggression, complex divorce, working with people with intellectual disabilities, or signaling elder and child abuse and domestic violence).
- Good knowledge of the Dutch language, both spoken and written.
