The role of ambulatory supporter is constantly evolving. You help people of all ages participate more independently in society. Expertise and affinity with the elderly target group are a plus.
What you do
- Create a plan together with the resident and their network
- Assist in planning and arranging support
- Provide support yourself where necessary and evaluate the results achieved
- Engage specialist support when needed
- Combine individual support with identifying and bundling requests for help to develop or implement a collective offer

- Flexible, creative, and eager to learn

- Minimum HBO (higher professional education) work and thinking level, such as social work, social community service
- Agrees with the stated secondment conditions

- Recent work experience in a similar position
- Fits within the team
- Can work both independently and in a team
- In possession of an SKJ registration
- Knowledge of the social domain, and knowledge and insight into the range of assistance and intervention methodologies
