A forensic psychiatric center provides care to patients. The institution does not have its own outflow chain to residential facilities with lower security levels, making collaboration with external chain partners important to properly shape a trajectory. In the transmural leave phase, patients go to follow-up facilities that are not always familiar with risk management and forensic acuity. You guide both the patient and colleagues of the follow-up facility in the transmural phase, on the way to probationary leave.
Your social skills play a central role in contact with vulnerable patients and in representation towards chain partners and within the center. You also bring knowledge of the 'social map', procedures, and frameworks within forensic care. You work as an outreach casemanager according to a largely independently structured work rhythm, actively present with chain partners to contribute to chain collaboration and optimization of patient care.
In addition, you are a team player and maintain contacts with internal departments to support the outflow perspective.
What you do
- A coordinating role prior to, during placement, and throughout the entire treatment trajectory at the follow-up setting (primarily FPA/RIBW/FBW) up to conditional discharge
- Contribute to finding a suitable follow-up institution in preparation for a transfer
- Participate in the intake interview between patient, mentor, and the official(s) at the follow-up institution to discuss mutual expectations and make agreements
- Maintain control over the implementation of the trajectory plan and risk management plan
- First point of contact for chain partners in crisis situations, but also for consultation regarding extended leave or other matters concerning treatment, and make decisions with the lead clinician
- Be a reliable discussion partner for patients
- Attend treatment plan meetings, as a representative on behalf of the clinic; for example, regarding extending or withdrawing freedoms, group rules, or requesting (additional) funds for resocialization in line with the lead clinician's vision
- Be responsible for requesting and checking legal annotations and ensuring the accuracy of the content, providing feedback if necessary
- Coach the follow-up setting (primarily FPA/RIBW/FBW) in patient interaction, but also the chain partner regarding forensic knowledge and risk thinking
- Work solitarily and independently
- Communicate at different levels, acting as a sparring partner and information source for the lead clinician, probation service, and the follow-up setting, taking into account the different interests when the patient resides in an institution with a reduced security level (risk thinking)
- Initiate and/or maintain network contacts
What they ask
- A completed HBO (higher professional education) degree in Social Work (SPH, SJD, MWD); or registered GGZ-agoge; or art. 3 BIG Act registered (social-psychiatric) nurse
- A high degree of independence, balanced with the ability to connect and integrate into different team contexts/environments
- Organizational skills, a practical approach and flexibility, a high degree of planning ability (also in terms of working hours and travel)
- Strong social skills: being accessible and reliable in communication, being a 'welcome guest', but also radiating authority and, where necessary, being able to persuade, engage people, and build connections between the clinic and follow-up institutions
- Expertise in patient issues, risk management, and forensic frameworks
- Good oral and written communication skills
- A B driving license and willingness to travel frequently
