A case manager is sought for a forensic facility for a temporary replacement. The role is crucial in the outpatient treatment of forensic clients, focusing on structure, safety, and recovery orientation.
What you do
- Conduct intakes and risk assessments, and develop care plans in collaboration with clients and lead therapists.
- Coordinate care in close consultation with the treatment team and external partners.
- Support the treatment responsible in shaping the treatment and contribute to relapse prevention.
- Work from a recovery-oriented care perspective, with an eye for the client's social context.
Profile
- Completed HBO (higher professional education) in nursing (AGB and BIG registered) or HBO agogist (registered with Registerplein).
- Affinity or experience with forensic psychiatry and the EPA target group.
- Ability to work independently and actively seek collaboration.
- Be resilient and maintain an overview in complex situations.
- Methodical and results-oriented in approach.
- Communicate clearly and coordinate well with clients, colleagues, and chain partners.
