As a Somatic Practice Assistant (POH-S), you guide patients in a forensic psychiatric clinic with chronic somatic conditions. In this clinic, patients reside for whom psychological issues, safety, and physical health are closely intertwined. Good somatic care is essential here for quality of life and the treatment process.
What you will do.
- Under the supervision of the general practitioner, you take on a significant part of the protocolized chronic care.
- You guide patients with diabetes mellitus, asthma/COPD, and within cardiovascular risk management (CVRM).
- You actively call patients for periodic check-ups, discuss results and lifestyle, signal complications or changes.
- You consult with the general practitioner in a timely manner when adjustments or additional examinations are needed.
- You maintain an overview of the care cycle: from check-up appointments and registration to follow-up actions.
- You build trust, remain clear about agreements, and tailor your guidance to the patient and their possibilities.
- You work independently within your area of expertise and have short lines of communication with the general practitioner(s), nurses, and other involved healthcare professionals.
- You support the administrative process of the medical service.
Profile
- You work carefully and guide patients in a clear, respectful manner in their care.
- You are comfortable with protocolized chronic care but also recognize when a patient needs more than the standard route.
- Experience in a complex care environment is welcome, but recent graduates are also invited to apply.
