Transition Coach
This assignment involves guiding young people up to 27 years old who are at risk of dropping out or have already dropped out of education. The goal is to reduce and prevent school dropout and to offer prospects to these young people. You will contribute to a promising future by guiding them towards an initial qualification or a suitable, sustainable job. The activities are carried out within a specialized team that works closely with network partners from education, care, and the social domain.
What you will do.
- Process absenteeism reports from young people aged 16 and older
- Conduct preventive interviews to prevent school dropout
- Advise schools on the approach to impending school leaving
- Identify and register (impending) dropout and early departure without an initial qualification
- Conduct discussions with young people, parents, and chain partners aimed at sustainable guidance
- Identify bottlenecks with young people and coach them towards appropriate follow-up steps
- Refer to appropriate help or support where necessary
- Conduct home visits
- Act as a fixed point of contact for young people and chain partners throughout the process
- Participate in case and youth consultations within education, care
- Manage the initiated trajectory and adjust it where necessary
- Stimulate cooperation with schools and chain partners
- Identify trends and contribute to innovative solutions within the field
- Participate in regular work meetings
- Report to the client or a supervisor
Profile
You will work independently on your own trajectories and manage the support provided. Daily execution takes place in close collaboration with colleagues within the team and with chain partners from education, care, and employment. Casuistry is discussed jointly in case and youth consultations where necessary.

- Minimum of 3 years of demonstrable work experience in guiding and coaching young people within the social domain and/or education
- Demonstrable work experience in managing individual trajectories for young people, involving collaboration with multiple stakeholders
- Demonstrable work experience in drawing up trajectory plans for young people and managing the process for these trajectories
Desired skills.
- Minimum of 3 years of demonstrable work experience in the past 6 years in guiding and coaching young people towards education, an initial qualification, and/or a sustainable job
- Minimum of 2 years of demonstrable work experience in the past 5 years with absenteeism, early school leaving, and/or the transition of young people within education
- Demonstrable work experience with complex youth cases, where the candidate has conducted interviews with young people and/or parents, identified bottlenecks, and initiated appropriate support or follow-up steps
- Demonstrable work experience in managing processes for youth trajectories, in collaboration with education and chain partners within the Social Domain
