- Collaboration between clients, their personal network, and the professional is central.
- A residential location provides 24-hour support to adults with an intellectual disability and/or additional issues.

What you will do.
- You guide residents in their normal lives, each according to their specific support needs.
- You draw up care plans together with the behavioral scientist and monitor their implementation with your core team.
- You listen carefully, value colleagues' input, and actively involve them in the treatment process.
- You have a coaching role towards your colleagues.

- Relevant diploma at minimum MBO level 4, such as Personal Counselor Social Care.
- Ability to speak open and clear language with clients and their families.
- Resourcefulness in coordinating care around clients.
- Maintaining connection with the network.
- Seeing a challenge in keeping clients' goals, care agreements, and care plans in order.
- Computer literate.
- Willingness to work irregular hours (day, evening, weekend, and sleeping shifts).
