A social district team works with families and residents from 0 to 100 years old. The professionals are keen to delve deep and contribute ideas about the right help. The team receives many applications concerning upbringing and growing up, such as ASS, domestic violence, child abuse, (fighting) divorces, and PGB questions. Questions about mental health care (GGZ), finances, debts, informal care, and WMO also arise. The team is large enough to work from specialisms and has various basic teams:
- an intake team that conducts a broad inquiry, focused on appropriate support
- a team that works with acute safety cases
- a specialized help and support team that provides basic help itself
- a specialized case management team
The team is looking for a case manager with expertise in safety.
What you do
- In consultation with colleagues, you dare to step outside the beaten path of laws and regulations if necessary to provide appropriate support for the family.
- As a case manager, you are a linchpin between the young person, their parent(s), care provider(s), school, and the young person's network.
- You ensure that appropriate help is delivered and, if necessary, look for alternative support to bridge gaps.
- You are critical and knowledgeable.
- You can work well under time pressure, clearly define questions, and do not lose sight of the overall picture.
- You are a true connector and know the (youth) care landscape well.

- Completed HBO (higher professional education) degree, focused on the function.
- Registration within the appropriate professional register (Youth Care SKJ).
- Works and thinks from the family's perspective.
- Analytical and pragmatic, able to connect with various collaborating parties.
- Capable of self-reflection.
- Empathetic and a good listener.
- A connector within families and their social network, within the team, and between collaborating partners.
- Possesses conversational skills and observational ability, and can connect with families in this regard.
- Good written skills.
- Able to make a good analysis in the hectic pace of the situation.
- Able to establish a relationship with those involved in a crisis-like context.
