General Practitioner Assistant

You will guide detainees with chronic conditions in a penitentiary institution.

• These are patients in a complex life phase, often facing multiple health and social problems simultaneously.
• You will have significant professional autonomy to make independent assessments, draw up care plans, and determine treatment policy in collaboration with the general practitioner.
• Focus on prevention, behavioral change, a healthier lifestyle, and providing information.
• You will contribute to the further development of care, for example, by translating national diabetes protocols into practice within the institution.
• This role is new within the medical service of the organization, offering ample room for personal interpretation.
• Possibility to combine this role with that of a judicial nurse, including daily and acute medical care within the institution.

What you will do.

• Take responsibility for your own patient group.
• Plan your own consultations and maintain an overview.
• Ensure detainees receive the right care at the right time.
• Actively collaborate with general practitioners, nurses, and other healthcare professionals.
• Sense the appropriate communication style needed when interacting with detainees.
• Share ideas and contribute to the further development of this new function.

What they ask

• HBO diploma as a General Practitioner Assistant Somatics (POH-S) or a nursing diploma (HBO-V or MBO-V with 5 years of work experience).
• Minimum of 2 years of experience as a General Practitioner Assistant.
• Knowledge of guiding patients with chronic conditions.
