Ambulant Counselor

You work independently and visit clients at home.
- You help clients and their relatives gain control over their daily lives.
- No day is the same, as every situation is unique.
What you do
- Provide practical support and a listening ear.
- Help clients gain control over issues related to health, work, finances, housing, and social contacts.
- Manage the case and be primarily responsible for the client's care file.
- Draw up care plans, monitor progress, evaluate results, and adjust where necessary.
- Look beyond the immediate request for help and identify what a client needs to move forward.
- Maintain an overview when situations are complex or involve multiple parties.
- Be the fixed point of contact for municipalities and other involved organizations.
- Maintain contact regarding indications, decisions, and the progress of guidance.
- Build trust, set boundaries, and motivate clients to get the most out of themselves.
- Be an important sparring partner for colleagues and contribute to the quality of guidance within the team.
- Ensure that clients and their relatives can count on appropriate support.
- Support people in reshaping their lives after brain injury.
What they ask
- HBO diploma (higher professional education) in Care and Welfare or Human and Society, such as SPH, Social Work, Nursing, or MWD.
- Interest in methodical work and competency-oriented guidance of clients with brain injury.
- Work experience in a similar position.
- Driving license B and own transport.
Profile
- You work with clients in a specific municipality and are part of one of two self-organizing teams.
- Each team consists of 12 to 14 colleagues who provide ambulant guidance or work within the day activity center.
- Although you work independently, the teams consciously seek each other out for collaboration.
- You have a lot of autonomy and guide clients at home where the support needs vary in complexity.
- Team meetings take place once every three weeks; low-threshold contact with colleagues is possible in between.
- Upon starting, you will receive a buddy and follow the 'My first 100 days' onboarding program.
