The approach focuses on strengthening self-reliance and restoring normal life, including assistance with registration, benefits, and daily activities. Together with participants, challenges and necessary steps are identified, and guidance is provided in various life areas such as mental health, judicial matters, and meaning-making. Additional help is sought when necessary.
You will work at a unique and pleasant residential location where residents work towards independence in a calm environment, contributing to a safe, stable, and stimulating living environment.
What you will do. As a counselor, you are central to the guidance process for participants with complex support needs. You manage the process from intake to outflow and aftercare, effortlessly switching between practical support and substantive guidance. Your responsibilities include:
- Drawing up, implementing, and adjusting personal pathway plans
- Providing practical support for matters such as benefits, finances, mail, housing, and daily activities
- Promoting stability and self-reliance among participants
- Coordinating with chain partners such as mental health services, addiction care, municipality, police, and probation services
- Coaching and supporting residential counselors in their daily work
- Timely signaling risks, adjusting, and escalating where necessary
- Contributing to the quality of care and policy at the location
Profile
- You have a completed HBO degree in Social Work, SPW, SPH, or a comparable study
- You have experience with recovery-oriented work and a complex target group
- Knowledge of Wmo pathways is a plus
- You are flexible and enjoy variety in your work
- You possess strong communication skills and great empathy
- You are a Christian (Catholic, Protestant, etc.) and can articulate what that means to you
