As a nursing casemanager in a FACT team, you will support clients with severe psychiatric disorders and addiction problems. You will be involved in psychiatric and somatic issues and manage your own client caseload. The focus is on recovery, stabilization, harm reduction, and safety, often for clients with a dual diagnosis (SMI and addiction). You combine nursing expertise with recovery-oriented care and work collaboratively in a multidisciplinary team to deliver high-quality, outreach care.
You coordinate care, conduct home visits, or meet clients at the FACT location. Daily consultations take place with the team and the primary care provider regarding client care. You are the point of contact for clients and a focal point for your expertise, making a difference with a creative and recovery-focused approach, even in complex situations.
What you do
- Signal changes in the clinical picture (psychiatric, somatic, and substance use) and anticipate them.
- Implement interventions focused on addiction care, such as motivational interviewing and lifestyle training.
- Perform nursing procedures, including administering (depot) medication and somatic screening.
- Maintain close contact with relatives, primary care providers, network partners, and social organizations.
Profile
- Completed Nursing degree (HBO-V) and BIG-registered.
- Demonstrable affinity and preferably experience with adults with SMI and addiction problems (dual diagnosis).
- Knowledge of somatics, pharmacology, and motivational interviewing.
- Decisive, resolute, and able to handle the unpredictability of outreach care.
- Possesses a sense of perspective and a good dose of humor.
- In possession of a driving license (and preferably own transport) for home visits.
Practical
- Gross monthly salary in scale 50 (€3560 – €4716) of the GGZ collective labor agreement.
- One-year contract with the prospect of a permanent contract.
- Good pension scheme and the possibility to fiscally advantageously purchase, for example, an (electric) bicycle or extra vacation days.
- 166 leave hours and 35 extra leave hours (based on full-time employment) in the form of a life-phase budget.
