A multidisciplinary practice is looking for a general practitioner to help build accessible, integrated primary care. You will work independently, but complex cases will be discussed and supported within a consultation-oriented team structure. The practice focuses on a diverse patient population, with particular attention to people with increased care needs.
What you will do.
- Provide broad general practice care, from acute and chronic care to prevention and health promotion.
- Follow up on patients with long-term or complex care needs, with attention to the coherence between physical health and psychological and social circumstances.
- Collaborate with an experienced general practitioner colleague, primary care psychologists, psychiatrists, social workers, and other care partners.
- In case of complex care questions, consult with the involved care providers and coordinate follow-up.
Profile
- Recognized general practitioner, starting or experienced.
- Enjoys collaborating on quality, person-centered primary care.
- Affinity with mental health care is welcome, but not a requirement.
- General practitioners in training who wish to start after their recognition are also welcome.
