You will work for a public organization that provides services to residents of multiple municipalities. You will contribute to meaningful work in an inspiring and challenging environment.
What you will do.
- You will be a case manager for complex (youth) cases covering multiple domains and/or involving safety risks.
- You will enhance safety within families/systems by creating an appropriate safety plan with them and chain partners.
- You will seek appropriate help and/or utilize support resources from the resident's social network.
- You will provide short-term assistance to those in need, through conversations or by phone.
- You will clarify residents' needs, contribute ideas, and assess what is required.
- You will develop concrete plans together with colleagues, families, and partners, involving valuable support resources from the social network.
- You will guide the approach to complex situations to increase safety and achieve sustainable solutions.
- The team organizes support for families/households/adults where the situation is complex, fragile, or unclear.
- The starting point is to limit the reliance on intensive support and, in the case of complex family situations, to prevent a judicial measure.
- The team does not focus on cases requiring forced assistance.
Profile
- You will work in a diverse team with various personalities and expertise.
- Structural support is provided by a behavioral scientist.
- The team has a high degree of autonomy.
