Trainee Practice Assistant Practice Nurse

A primary care organization is looking for a (trainee) practice assistant/practice nurse for a general practitioner's practice. This role prepares for the retirement of a colleague, with a focus on a good induction period and careful handover. The organization supports general practitioners in business operations, chronic care, mental health care, digitalization, telephony, and innovation.

What you will do.

• Perform medically-substantive care tasks delegated by the general practitioner to patients with chronic conditions such as DM, Asthma/COPD, cardiovascular problems, lifestyle, and/or nutrition.
• Independently hold consultations, including providing information, education, and instructions to patients.
• Conduct home visits.
• Perform triage and, if necessary, physical examinations or medical technical procedures.
• Refer to other first-line healthcare providers and, in consultation with/under the responsibility of the general practitioner, to second-line care.
• Manage your own patient administration.
• Play an important role in quality assurance.

What they ask

• Be resilient, have experience and affinity with patients with chronic diseases.
• Hold a nursing diploma (mbo/hbo) or doctor's assistant diploma and have at least 2 years of work experience as a doctor's assistant.
• Possess good social and communication skills, focused on collaboration with general practitioners and colleagues.
• Service-oriented towards the patient, integer, strong organizational skills, independent, decisive.
• Able to handle resistance, solution-oriented, and possess persuasive power.
• Knowledge of the local social map is a plus.
