You guide and improve the quality of the patient network within a specialized department of a forensic care institution. The goal is to improve the treatment process and reduce the risk of recidivism. You contribute to the treatment of patients by involving the existing network as much as possible in the treatment and future plans. Where necessary and possible, the social network is expanded to reduce the chance of social isolation.
What you do
- Maintain contact with the professional and social network of patients (family, friends, partners, volunteers, etc.) and encourage them to make a positive contribution to the treatment.
- Organize multidisciplinary treatment consultations with the network.
- Be aware of the patients' history and, based on a systemic vision, form a judgment regarding treatment and risk management.
- Support patients and their network in breaking old destructive patterns of repetition.
- Prepare visit reports, report findings, contribute to statuses, and coordinate findings with involved disciplines.
- Work closely with the primary therapist and the department team leader.
- Be jointly responsible for coaching group leaders in the execution of treatment within the department.
- Play an active role in connecting the patient, the network, patients among themselves (e.g., friendly or partner relationships), and the treatment team, for example, by arranging leaves to the network.
Profile
- HBO-level education (higher professional education); additional training in systemic work is a plus.
- Education included in the list of consultation-registering professions of the Care Performance Model, OR willing to obtain the necessary education or registration to comply.
- Affinity with the target group and understanding the necessity of forced assistance.
- Ability to handle various interests well within frequent internal and external contacts.
- Excellent oral and written communication skills.
- Competencies such as stress resistance, systemic thinking, proactivity, flexibility, and patience.
- Openness to other cultures, habits, and customs.
- In possession of a B driving license.
