Mobile Shared Help Employee Care & Casemanagement

East FlandersOn siteHours negotiable>12months

An organization in mental health care for children and young people offers mobile assistance in three forms: early help for mild to moderate psychological needs, rapid help in suddenly aggravated crisis situations, and shared help for multiple and complex problems. This position focuses on the Shared Help cluster, specifically the Care and Casemanagement modules.

What you do

As a mobile Shared Help employee (Care):

  • Provide guidance to children and young people up to 18 years old with complex care needs across various life domains.
  • Offer support for psychological and psychiatric problems for which the existing care offer is insufficient or trust in assistance is under pressure.
  • Prevent and repair breaks in the care trajectory, so that care management is regained by:
  • Providing individual trajectories for children/young people and their context.
  • Supporting facilities and aid workers already involved.
  • Investing in effective collaboration, care, and trajectory coordination with social services and centers.

As a mobile Shared Help employee (Casemanagement):

  • Follow up on stalled trajectories of minors with (suspected) psychological/psychiatric problems.
  • Organize and coordinate multilateral, intersectoral, and trust-building consultations between minors, family, supportive figures in the context, involved aid (at least one mental health partner), and other partners (e.g., school), prioritizing the needs of the client system.
  • Coordinate/co-facilitate feasible, individual aid worker trajectories.
  • Support the minor, their context, and involved (care) partners in developing the care trajectory.
  • Coordinate the care trajectory and follow up on agreements made in the interim.
  • Think creatively and out-of-the-box in consultation with all stakeholders in the active search for the most suitable care.
  • Build an (aid) network around the minor and their context.
  • Critically monitor, evaluate, and adjust the commitment and operationalization of aid and services, and the progress of the network process for the minor and their family.
  • Prepare reports during consultations and consolidate agreements made.
  • Ensure confidentiality of information and perform tasks in light of applicable regulations regarding professional secrecy.
  • The guidance runs from start to finish according to a treatment line. Treatment goals and the care trajectory are evaluated and adjusted at various moments.
  • Be part of the same multidisciplinary regional team with a fixed regional coordinator and network psychiatrist.
  • Participate weekly in team meetings led by the regional coordinator, focusing on exchange and case discussions.
