Temporary assignment to replace an employee.
- You will join a dynamic, multidisciplinary team within a psychiatric hospital and be responsible for patients in addiction care.

What you will do.
- Conduct a thorough analysis of the patient's social situation and determine the most appropriate support for their needs.
- Guide patients and their environment towards a feasible and safe discharge situation, taking into account both the home situation and the hospital context.
- Work closely with the multidisciplinary team to strengthen the psychosocial well-being of patients and their environment.
- Apply various guidance methodologies, such as individual interviews, to provide tailored support to patients.
- Register all interventions to optimize internal operations and patient follow-up.
- Build a strong network with external partners, coordinate actions in individual files, and ensure smooth referrals.
Profile
- Bachelor's degree in social work or social nursing with thorough knowledge of the healthcare landscape.
- Proficient in Microsoft Office and quick to learn new IT applications.
- Well-informed about social provisions, rights research, social map, and legislation.
- Clear and empathetic communication in Dutch, with strong intercultural and administrative skills.
- Work autonomously, set priorities, and maintain an overview in complex situations.
- Contribute ideas, seek solutions, and flexibly switch between the patient, network, and care team.
