POH Elderly Care

What you will do. As a POH Elderly Care professional, you will engage with elderly individuals and their families at an early stage to offer appropriate assistance. This involves patients with physical, cognitive, and psychological fragility due to aging, such as (early-stage) dementia, loneliness, or physical discomforts.
- Map the patient's situation and draw up a care plan.
- Build a relationship of trust with the elderly, their social environment, and other involved healthcare providers.
- Take the lead in the process and involve the immediate environment, the social domain, and other care disciplines.
- Conduct consultations, both at the practice and at the homes of elderly individuals, including physical examination.
- Coordinate and manage care, including drawing up individual care plans, multidisciplinary consultations, and guidance for chronic care.
- Maintain and expand a network within the social domain and connect 0th, 1st, and 2nd line care.
- Monitor medication safety, guide medication, and facilitate advance care planning for the general practitioner.
Profile
The POH Elderly Care role is evolving, and you will play an important part in its implementation.
- Good organizational and social skills.
- Enjoys pioneering and optimizing processes.
- Proactive and enthusiastic attitude, able to motivate others.
- Raises awareness for the care of (vulnerable) elderly.
- Possesses a driving license and own transport (due to home visits).
What they ask
- Completed POH Elderly Care training at NLQF-6 level.
- HBO-V in combination with the POH Elderly Care training NLQF-6 or the desire to follow this training.
- POH Somatics HBO-V trained in combination with the POH Elderly Care training or the willingness to follow it (MBO trained nurses have an NLQF-6 level education, or are already admitted to the training).
- Nurse Specialist.
- Physician Assistant with additional training and/or experience in Elderly Care.
- Minimum of 2 years of relevant work experience within elderly care.
