A healthcare organization is looking for a team member for a mobile psychiatric team for long-term care.
- The team provides treatment and guidance in the home situation, aiming to avoid or shorten hospital admissions.
- The care offered is aimed at adults with severe and complex psychiatric problems.
What you do
- Provide guidance and treatment in the home environment of adult clients (18+) with psychiatric vulnerability. The target group is characterized by a mix of problems in terms of somatic, mental health, and social/societal functioning.
- Build a trusting relationship with the client and their professional and non-professional network. If no network is present, focus on developing it to achieve tailored care and continuity of care.
- Promote expertise in psychosocial and mental health care among the various parties involved with the client, by fostering care coordination, participating, and offering coaching where needed/desired.
Profile
- Bachelor's degree in social/psychological counseling, social work, or orthopedagogy.
- Minimum 2 years of experience in mental health care.
- Communicative and possessing self-insight. Able to report clearly to the multidisciplinary team.
- Strong in planning and organizing, pragmatic, and proactive.
- Stress-resistant and flexible.
- Able to deal with diversity.
