Casemanager OGGZ

• You provide expert support to both the team and to clients and their relatives, helping them cope with the challenges that dementia brings
• You develop and implement individual care plans, working closely with a multidisciplinary team and ambulatory counselors to ensure optimal care
• You provide clear information and education to clients and informal caregivers about dementia, available services, and support options, guiding them in making well-informed choices
• You maintain intensive contact with general practitioners, geriatricians, district nurses, fellow case managers, the municipality, and other care institutions to create a coherent and effective care network
• You closely monitor the progress of clients, regularly evaluate care plans, and adjust them based on changing needs and circumstances. All care interactions and decision-making are carefully and timely documented

What they ask

• A higher professional education (HBO) nursing diploma and a valid BIG registration
• A post-HBO diploma in dementia case management or willingness to obtain it (training starts in October)
• Experience with indicating and applying for CIZ indications
• A driver's license
• The ability to deal with behavior appropriate to the OGGZ target group
• A Christian background (Catholic, Protestant, etc.) and willingness to explain what this means to you

• You work regular office hours.
• In the initial period, you will be trained in the district.
• During the training (if not yet completed), you will work in the district, which may temporarily involve irregular shifts.
• It is possible to combine this position with work as a nurse in district nursing.
