A government organization is gradually building a new municipal access point for Youth Care and Wmo Guidance. For this, we are looking for experienced consultants who want to contribute to the development of future-proof access and, at the same time, support residents with appropriate help and support. You will join a team under construction. In addition to performing substantive tasks, you will actively contribute to the development of processes, working methods, and cooperation agreements.
The complexity of the issues requires professional judgment, an integral perspective, and good cooperation with chain partners.
What you will do.
- Conduct conversations with residents about support questions regarding self-reliance and participation.
- Investigate the request for help and assess the necessity of Wmo guidance.
- Indicate and decide on customized provisions within the Wmo.
- Stimulate self-reliance, the use of the network, and general provisions where possible.
- Manage the deployed support and monitor results.
- Collaborate with care providers, network partners, and other involved professionals.
- Signal changes in situations and adjust in a timely manner where necessary.
Profile
- Completed HBO education, for example Social Work, SPH, MWD, Occupational Therapy, or Nursing.
- Demonstrable experience as a Wmo consultant.
- Knowledge of the Wmo, indication processes, and relevant laws and regulations.
