What you will do.
- You will start in one team, with the possibility to grow into another regional team in due course, in good consultation.
- You are skilled in collaboration and connecting with clients, their relatives, and collaborating organizations.
- You manage your own caseload and know your clients well.
- You draw up treatment plans and consult with colleagues on the best approach for this vulnerable target group, almost always with multi-problematic issues.
- You are the linchpin and ensure that the care around the client is organized as well as possible.

- An enthusiastic, independently working HBO-qualified nurse or supervisor.
- Meticulous, with good social and written skills.
- Someone who enjoys teamwork and derives satisfaction from organizing everything as well as possible for the team and clients.
- A completed HBO nursing degree (BIG registration) or an Agogist with a mental health agogist registration in the registerplein, eligible for billing within the ZorgPrestatieModel (ZPM).
- Experience as a nurse or supervisor within mental healthcare (GGZ).
- Easily establishes contact with clients, therapists, and external chain partners.
