A healthcare organization is looking for a mental health expert for the social district teams of a municipality. You will work with a target group with complex psychiatric problems.
What you do
- signal mental health issues in the district;
- assess and triage caseloads within the district team, focused on recognizing mental health issues;
- conduct case management for individuals with mental health issues in the district;
- refer individuals with mental health issues to appropriate treatment/support;
- maintain contact with, among others, the district police officer, district network, mental health organizations, general practitioners, POHs, and (F)ACT teams;
- coach colleagues on mental health issues and cases.
What they ask
- Completed relevant education (MWD/SPH/HBO Nurse with BIG Registration);
- Preferably experience with EPA clients;
- Affinity and experience with guiding and treating clients in crisis situations;
- Business-oriented mindset and a sense of co-responsibility for the overall organizational outcome;
- Collegial, flexible, strong communication skills, and a networker;
- High degree of independence, decisive, and solution-oriented, even under (time) pressure.
Practical
- A positive Certificate of Conduct (VOG) is required.
- Reference checks and diploma verification are part of the application procedure.
