Ambulatory Support Worker

• You assist with financial matters, relationship problems, and other complex issues that may arise simultaneously, such as work, income, and health.
• You create a personalized care plan, adapted to the client's wishes and capabilities, and are responsible for its implementation and adjustment.
• You motivate and stimulate clients, allowing them to maintain control, unless temporary intervention is necessary.
• You listen attentively and ask the right questions to uncover the root cause of the problems.
• You act as a confidant for the client and their environment, considering all involved parties.
• Patience, humor, and empathy are essential in this role.

• You hold a higher professional education (hbo) diploma in the field of Care and Welfare or Human and Society, such as SPH, Social Work, Pedagogy, or MWD.
• You have an interest in methodical work and competency-based guidance for clients with brain injury.
• You possess a B driving license and your own transportation.

Practical aspects.

• Monday is a fixed working day for team meetings.
• Availability on Friday is preferred.
• The remaining working days are flexibly scheduled in consultation with colleagues and clients.
• You work in an enthusiastic and dynamic team with a buddy system for onboarding and support.
• There is a group chat and short lines of communication for mutual contact.
• You are always welcome to drop by the day activity center.
• A Certificate of Good Conduct (VOG) is part of your employment.
